Companies issued citations for deadly oil explosion

An explosion near an oil well last August that killed three workers has caused Wyoming safety officials to issue 19 citations to several companies involved in the deadly explosion.

The names of the companies and amounts of the fines has not been released. The three workers were installing pipe between two storage tanks that separate water and oil when the explosion occurred. The explosion caused a fire at an oil well, which burned 10 acres before the fire department got it under control.

In the past decade, Wyoming has consistently ranked among the highest in workplace fatalities. A state-hired epidemiologist reported that Wyoming employers consistently fail to enforce safety rules for its employees.

Explosion in waste plant injuries one

A major explosion in a West Palm Beach solid waste plant Tuesday morning injured one worker and blew off the plant’s heavy metal doors.

The cause of the explosion is currently under investigation. A spokesman for the city says that three employees were in the building doing routine maintenance and cleaning when the explosion occurred. So far, investigators are not sure if the work employees were performing sparked the explosion.

The one injured worker, who is the chief operator of the waste treatment plant, was taken to the hospital for observation and evaluation. He was released the next day.

Gas line explosion injuries four

Two volunteer firefighters and two contractors were injured yesterday in West Haverstraw, New York when the contractors working with a phone company hit a natural gas line while digging.

The ruptured gas line leaked into the sewer line and caused a major explosion that leveled a townhouse. No one was inside the house at the time of the explosion. Residents were only given minutes warning to evacuate before the explosion occurred.

The powerful force of the explosion rocked volunteer firefighters nearly 30 feet back. They suffered first and second degree burns and a few broken bones. The contractors suffered minor injuries.

Father and daughter hurt in home explosion

A home explosion, believed to have been caused by a propane leak, completely destroyed a home near Harrisburg, South Dakota on Monday.

The two people inside the home at the time, a 49 year-old father and his 21 year-old daughter, are being treated for injuries. According to authorities, the daughter was able to drive her father to the hospital. He is said to have been airlifted to a Minneapolis burn center. Her condition is unknown.

The explosion, which occurred around 3 pm, continued to burn past 7 pm. All that is left of the ranch-style home is the attached garage and part of the front porch.

Explosion sends manholes flying, one injured

A 51 year-old man has been charged with fourth-degree endangering the public health or environment and leaving the scene of a property damage accident after he hit a gas pump with his car.

The collision caused gasoline to leak, causing an explosion that sent manhole covers flying into the air. Hundreds of residents in Hudson Falls, New York were forced to evacuate. More than 1,000 gallons of gas leaked into the Hudson Falls’ and and Fort Edward’s sewer system. The explosion damaged nearby homes and businesses when manhole covers were blown off.

One man was injured by the explosion and sprained his knee when he was thrown into the air.

Gas leak may have caused dangerous explosion

The cause of a house explosion that critically injured three people last week is currently under investigation by Kansas Gas Service and the local fire department.

The family of three, including a two year-old child, are in the hospital in serious and critical condition. Investigators have not been able to speak with the house’s occupants due to their conditions. Kansas Gas Services said they found a gas leak in a main gas line not far from the residence, but it cannot confirm if it caused the explosion.

Relatives told investigators that the family relied on space heaters to keep warm, but so far no one has been able to confirm if heaters played a role in the explosion.

House explosion leaves one seriously injured

A natural gas leak is believed to have caused a serious house explosion in New Albany, Indiana Monday night that injured one person.

The homeowner said her grandson had gone outside to smoke a cigarette and came back inside moments before the explosion occurred. He is currently in University Hospital in Louisville with second-degree burns on the front of his body.

The explosion blew off the roof of a closed-in porch and debris fell 50 feet. Investigators believe the gas stove is the source of the explosion but are searching for where the gas leak originated. This is the third house explosion in New Albany since November, according to the New Albany fire chief.

Settlement reached in mine explosion

A 2010 West Virginia mine explosion that killed twenty-nine workers has resulted in the mine’s owners, Massey Energy, being forced to pay a record $210 million.

$46.5 million of the settlement will go towards the miners’ families for criminal restitution, $35 million in penalties for safety violations, and $128 million to improve safety, training, and research. A report by the Department of Labor’s Mine Safety and Health Administration listed 369 violations in the mine with at least a dozen contributing to the deadly explosion.

The MSHA and federal prosecutors believe that Massey Energy is guilty of criminal activities such as obstructing federal regulators, lying to investigators about health and safety problems, and violating federal mine safety laws.

PG&E fined $38 million for explosion

The California Public Utilities Commission has fined PG&E Co. $38 million for a fatal explosion that occurred in 2008 in Rancho Cordova.

The explosion, which was caused by poor repairs that caused gas to leak, killed one and injured five people. PG&E has also been blamed for a bigger explosion in 2010 that killed eight people, injured 58, and leveled a neighborhood in San Bruno, California. 38 homes were destroyed and 70 others were damaged when a gas line exploded.

PG&E could see their fine for the San Bruno accident reach as much as $800 million. The company says it plans to spend $2.2 billion in the next two years in order to improve pipeline safety.

Family awarded $27M for deadly propane explosion

An Indianapolis jury has awarded a family $27 million after a deadly propane explosion left one person dead and four others injured, including two two year-old children.

The explosion occurred in May 2004.  The jury decided that the South Central Indiana Rural Electric Membership Corp., RushShelby Energy Rural Electric Cooperative, and SCI Propane LLC, should compensate the family. However, it decided that the utility companies were only 65% liable and that the now deceased homeowner was 35% responsible.

The family will then receive 65% of the $27 million, or about $17.5 million. The utility companies have the option to appeal the decision.
