An explosion at a refinery in Utah which damaged 10 houses was caused by a burst pipe, according to an investigation conducted by the US Chemical Safety Board. The pipe ruptured, spraying hydrogen gas towards a nearby heater, which led to the explosion.
The report cites the rupture as a “catastrophic failure,” and they are currently attempting to determine what caused the initial break. The pipe was 10 inches in diameter and was carrying 630 pounds of pressurized hydrogen when the failure occurred.
