Manufacturing plant fined $80K for explosion

A Tennessee manufacturing plant has been fined by the Occupational Safety and Health Administration $80,000 following a fatal explosion that occurred in May. OSHA found twenty-three violations in its investigation.

Hydrogen gas was leaking from an underground pipe, which caused the explosion. Three employees were killed and two were seriously injured.

The manufacturing plant, Hoeganaes, has experienced two other explosions this year in its other plants around the country. OSHA fined it $42,900 after two explosions that occurred in January and March and caused two deaths. Hoeganaes has contested these fines and has about three weeks to respond to OSHA’s newest fines.

Chicago home explosion injuries two

A home in Chicago’s South Side exploded in the early morning hours, leaving two of its inhabitants in critical condition with second-degree burns.

The cause of the explosion is still under investigation. Fire officials say a neighbor was working on plumbing in his basement at the time of the explosion, however it is unclear if the events are connected.

Neighbors say the explosion rocked their houses and blew out several windows. The house was completely leveled and flames reached as high as the tallest trees. Some neighbors said they smelled natural gas a few hours before the explosion occurred; no one reported the smell.

Many houses surrounding the home were damaged by the flames that spread to their roofs.

Two sue for injuries caused by propane explosion

Two people in Muskogee, Oklahoma who were critically injured by a home propane explosion have filed a lawsuit against five companies they believe are responsible.

In September, employees of W.E. Allford Propane finished installing a propane tank in the home and showed them how to hook it up to the propane heater, purchased from Lowe’s, inside. That night, one of the friends tried to turn it on by pressing the electric-ignite button. The tank immediately exploded and threw him back.

The home was completely destroyed and both suffered serious injuries. They are seeking compensation for their medical expenses, loss of property, loss of income, permanent and total disability, scarring, and disfigurement.

Stuntman killed in on-set explosion

A stuntman was killed late last week in an explosion while filming for the action-adventure film Expendables 2, a sequel starring Sylvester Stallone, Bruce Willis, Arnold Schwarzenegger, and Chuck Norris.

The accident is currently under investigation, but witnesses say something went tragically wrong while filming a scene with a rubber boat and a staged explosion. Another stuntmen was seriously injured but is now in stable condition. The crew was filming on the Ognyanovo dam in Bulgaria.  None of the actors were nearby during the incident.

Filming for the main unit has not been delayed, but the second unit crew has suspended filming while the investigation is underway.

Massey Energy blamed for coal mine explosion

The deadliest U.S. coal mine explosion in the past 25 years that left 29 workers dead and four unaccounted four has been blamed on the mine’s owners, Massey Energy.

The explosion, which occurred in April of last year in West Virginia, was found to be caused by an obscured methane gas sensor. The United Mine Workers, which recently published a report of its investigation, claims that Massey Energy was responsible for maintaining the safety of the mine and its workers at all times.

The report claims that if the methane gas sensor was not covered by a ventilation curtain, the sensor would have been able to detect high levels of the gas.

Man causes explosion and six-alarm fire

A 28 year-old man caused an explosion, and subsequently a six-alarm fire, in a failed suicide attempt. The man was living with a relative in an apartment complex in Medford, Massachusetts when he rigged the natural gas line and caused it to ignite.

Nearly 50 people have been displaced after fire, smoke, and/or water damaged left 24 apartments uninhabitable. The fire burned for over eleven hours before it was contained and ten people were injured as a result.

The man behind the explosion is currently in the hospital suffering from third-degree burns. He is expected to face charges of attempted murder and arson once his medical conditions improve.

Furnace believed to have caused explosion

A newly installed furnace is suspected to be the cause of a deadly explosion in a residential Kansas home. Investigators say that the furnace was installed just days before it exploded and killed a woman.

The 53 year-old homeowner along with her 23 year-old daughter and her friend are still in the hospital. The mother is in critical condition and her daughter and friend are said to be in stable condition. Emergency responders say the explosion leveled the house and threw debris more than a hundred feet away.

The Reno County Sheriff’s Department believes a propane tank leaking gas is what ignited the furnance.

PG&E blamed for deadly gas line explosion

Pacific Gas & Electricity Co. has been blamed for the gas line explosion that destroyed over a hundred homes in a San Bruno neighborhood and injured dozens of residents last year. A federal investigation by the National Transportation Safety Board says PG&E is to blame after, for nearly half a century, the company failed to properly maintain the gas line.

The investigative report by NTSB says that the company installed substandard pipes approximately 50 years ago. PG&E did not conduct proper visual inspections or pressure tests and therefore never noticed the problem. The NTSB claims that the company also placed a maximum gas pressure limit on the line that was too high for the type of pipe installed.

The NTSB also blames PG&E for allowing the gas to leak for nearly an hour longer than it should have if the company’s control room operators informed 911 of the source of the rupture.

House fire in Central Texas caused by propane tank explosion

A house fire between San Marcos and Kyle in Central Texas occurred after a propane tank exploded. This house fire also caused multiple other grass fires to occur in the area.

The fire departments in Kyle and San Marcos as well as the Wild Land Task Force were called in to help battle the brush fires. The fire was contained earlier today and no one was injured in the house fire. Everyone was evacuated from the home before the fire broke out.

Louisiana Exxon faces cited after explosion

A Louisiana Exxon has been cited for  safety violations after a refinery explosion.

The refinery now faces a total of $126,000 in fines for the safety violations that left their workers exposed to fires and explosions. This refinery, located in Baton Rouge, is the country’s second largest refinery. It was cited for 20 serious violations and 2 other violations after the  investigation.

Exxon is the world’s largest company according to their market capitalization. Officials with Exxon hope to have some of the citations of these withdrawn at the next meeting.
