Petroleum giant BP has been fined an unprecedented $87 million by the US government for the 2005 explosion at their Texas City refinery which killed 15 and injured 170 others. Furthermore, the Occupational Safety and Health Administration noted that the company still has a “serious systemic safety problem” four years after the accident occurred.

OSHA originally fined the company $21.3 million shortly after the incident originally occurred, and BP has shelled out $2 billion in settlements to people injured in the accident and the families of those killed. The company immediately promised to correct the safety failures which caused the accident, but four years later, OSHA says those corrections have yet to be made.
