Explosion sends manholes flying, one injured

A 51 year-old man has been charged with fourth-degree endangering the public health or environment and leaving the scene of a property damage accident after he hit a gas pump with his car.

The collision caused gasoline to leak, causing an explosion that sent manhole covers flying into the air. Hundreds of residents in Hudson Falls, New York were forced to evacuate. More than 1,000 gallons of gas leaked into the Hudson Falls’ and and Fort Edward’s sewer system. The explosion damaged nearby homes and businesses when manhole covers were blown off.

One man was injured by the explosion and sprained his knee when he was thrown into the air.

Settlement reached in mine explosion

A 2010 West Virginia mine explosion that killed twenty-nine workers has resulted in the mine’s owners, Massey Energy, being forced to pay a record $210 million.

$46.5 million of the settlement will go towards the miners’ families for criminal restitution, $35 million in penalties for safety violations, and $128 million to improve safety, training, and research. A report by the Department of Labor’s Mine Safety and Health Administration listed 369 violations in the mine with at least a dozen contributing to the deadly explosion.

The MSHA and federal prosecutors believe that Massey Energy is guilty of criminal activities such as obstructing federal regulators, lying to investigators about health and safety problems, and violating federal mine safety laws.

Manufacturing plant fined $80K for explosion

A Tennessee manufacturing plant has been fined by the Occupational Safety and Health Administration $80,000 following a fatal explosion that occurred in May. OSHA found twenty-three violations in its investigation.

Hydrogen gas was leaking from an underground pipe, which caused the explosion. Three employees were killed and two were seriously injured.

The manufacturing plant, Hoeganaes, has experienced two other explosions this year in its other plants around the country. OSHA fined it $42,900 after two explosions that occurred in January and March and caused two deaths. Hoeganaes has contested these fines and has about three weeks to respond to OSHA’s newest fines.

Chicago home explosion injuries two

A home in Chicago’s South Side exploded in the early morning hours, leaving two of its inhabitants in critical condition with second-degree burns.

The cause of the explosion is still under investigation. Fire officials say a neighbor was working on plumbing in his basement at the time of the explosion, however it is unclear if the events are connected.

Neighbors say the explosion rocked their houses and blew out several windows. The house was completely leveled and flames reached as high as the tallest trees. Some neighbors said they smelled natural gas a few hours before the explosion occurred; no one reported the smell.

Many houses surrounding the home were damaged by the flames that spread to their roofs.

Stuntman killed in on-set explosion

A stuntman was killed late last week in an explosion while filming for the action-adventure film Expendables 2, a sequel starring Sylvester Stallone, Bruce Willis, Arnold Schwarzenegger, and Chuck Norris.

The accident is currently under investigation, but witnesses say something went tragically wrong while filming a scene with a rubber boat and a staged explosion. Another stuntmen was seriously injured but is now in stable condition. The crew was filming on the Ognyanovo dam in Bulgaria.  None of the actors were nearby during the incident.

Filming for the main unit has not been delayed, but the second unit crew has suspended filming while the investigation is underway.

Massey Energy blamed for coal mine explosion

The deadliest U.S. coal mine explosion in the past 25 years that left 29 workers dead and four unaccounted four has been blamed on the mine’s owners, Massey Energy.

The explosion, which occurred in April of last year in West Virginia, was found to be caused by an obscured methane gas sensor. The United Mine Workers, which recently published a report of its investigation, claims that Massey Energy was responsible for maintaining the safety of the mine and its workers at all times.

The report claims that if the methane gas sensor was not covered by a ventilation curtain, the sensor would have been able to detect high levels of the gas.

Man causes explosion and six-alarm fire

A 28 year-old man caused an explosion, and subsequently a six-alarm fire, in a failed suicide attempt. The man was living with a relative in an apartment complex in Medford, Massachusetts when he rigged the natural gas line and caused it to ignite.

Nearly 50 people have been displaced after fire, smoke, and/or water damaged left 24 apartments uninhabitable. The fire burned for over eleven hours before it was contained and ten people were injured as a result.

The man behind the explosion is currently in the hospital suffering from third-degree burns. He is expected to face charges of attempted murder and arson once his medical conditions improve.

Names released of those killed in Wyoming explosion

The state has now identified the names of the men that were killed in a Wyoming workplace oil explosion.

Officials from the Wyoming Occupational Safety and Health Administration have identified the victims that were apart of the fatal oilfield explosion. Two men worked for Double D Welding and the other worked for Wild West Construction.

The explosion occurred at Samson Resources Co. Officials are still at the scene at this time and the Wyoming OSHA has 180 days to determine the cause of the explosion and see if any workplace violations were attributed to the cause. Workers at the plant were installing a fuel line earlier in the week, but officials with the plant have said that this was not the cause.

Explosion causes car wash to level out

An explosion in Lancaster caused a car wash to level out when the blast occurred at 3:30 a.m.

The explosion was so powerful it caused chunks of the building to fly across the street and knocked out windows of nearby mobile homes. A number of homes in the mobile park were damaged in the explosion.

The car wash was near a propane site, but it is unknown at this time whether or not this play a role in the blast. An investigation into the explosion is underway at this time.

Over 20 injured in massive California gas explosion

A massive gas explosion in San Bruno, California has injured over 20 people and destroyed more than 50 homes.

According to emergency officials, the gas explosion occurred in a neighborhood near the San Francisco airport. The explosion occurred at 9 p.m. on Thursday after a gas line erupted in the area, according to officials from the Pacific Gas and Electric. The cause of the rupture is currently being investigated.

Overnight, the flames spread across ten acres of land and at least 53 house have suffered severe damage, while over 120 have some damage due to the flames.

Over 100 people in the area had to evacuate their homes and stay in shelters while nearly 200 firefighters fought the fast moving flames. At least three people have been killed and over 20 have been taken to the hospital to be treated for their injuries. A state of emergency has been called in the San Mateo County area.
