Explosion sends manholes flying, one injured

A 51 year-old man has been charged with fourth-degree endangering the public health or environment and leaving the scene of a property damage accident after he hit a gas pump with his car.

The collision caused gasoline to leak, causing an explosion that sent manhole covers flying into the air. Hundreds of residents in Hudson Falls, New York were forced to evacuate. More than 1,000 gallons of gas leaked into the Hudson Falls’ and and Fort Edward’s sewer system. The explosion damaged nearby homes and businesses when manhole covers were blown off.

One man was injured by the explosion and sprained his knee when he was thrown into the air.

Settlement reached in mine explosion

A 2010 West Virginia mine explosion that killed twenty-nine workers has resulted in the mine’s owners, Massey Energy, being forced to pay a record $210 million.

$46.5 million of the settlement will go towards the miners’ families for criminal restitution, $35 million in penalties for safety violations, and $128 million to improve safety, training, and research. A report by the Department of Labor’s Mine Safety and Health Administration listed 369 violations in the mine with at least a dozen contributing to the deadly explosion.

The MSHA and federal prosecutors believe that Massey Energy is guilty of criminal activities such as obstructing federal regulators, lying to investigators about health and safety problems, and violating federal mine safety laws.

Family awarded $27M for deadly propane explosion

An Indianapolis jury has awarded a family $27 million after a deadly propane explosion left one person dead and four others injured, including two two year-old children.

The explosion occurred in May 2004.  The jury decided that the South Central Indiana Rural Electric Membership Corp., RushShelby Energy Rural Electric Cooperative, and SCI Propane LLC, should compensate the family. However, it decided that the utility companies were only 65% liable and that the now deceased homeowner was 35% responsible.

The family will then receive 65% of the $27 million, or about $17.5 million. The utility companies have the option to appeal the decision.

Manufacturing plant fined $80K for explosion

A Tennessee manufacturing plant has been fined by the Occupational Safety and Health Administration $80,000 following a fatal explosion that occurred in May. OSHA found twenty-three violations in its investigation.

Hydrogen gas was leaking from an underground pipe, which caused the explosion. Three employees were killed and two were seriously injured.

The manufacturing plant, Hoeganaes, has experienced two other explosions this year in its other plants around the country. OSHA fined it $42,900 after two explosions that occurred in January and March and caused two deaths. Hoeganaes has contested these fines and has about three weeks to respond to OSHA’s newest fines.

Chicago home explosion injuries two

A home in Chicago’s South Side exploded in the early morning hours, leaving two of its inhabitants in critical condition with second-degree burns.

The cause of the explosion is still under investigation. Fire officials say a neighbor was working on plumbing in his basement at the time of the explosion, however it is unclear if the events are connected.

Neighbors say the explosion rocked their houses and blew out several windows. The house was completely leveled and flames reached as high as the tallest trees. Some neighbors said they smelled natural gas a few hours before the explosion occurred; no one reported the smell.

Many houses surrounding the home were damaged by the flames that spread to their roofs.

Stuntman killed in on-set explosion

A stuntman was killed late last week in an explosion while filming for the action-adventure film Expendables 2, a sequel starring Sylvester Stallone, Bruce Willis, Arnold Schwarzenegger, and Chuck Norris.

The accident is currently under investigation, but witnesses say something went tragically wrong while filming a scene with a rubber boat and a staged explosion. Another stuntmen was seriously injured but is now in stable condition. The crew was filming on the Ognyanovo dam in Bulgaria.  None of the actors were nearby during the incident.

Filming for the main unit has not been delayed, but the second unit crew has suspended filming while the investigation is underway.

Massey Energy blamed for coal mine explosion

The deadliest U.S. coal mine explosion in the past 25 years that left 29 workers dead and four unaccounted four has been blamed on the mine’s owners, Massey Energy.

The explosion, which occurred in April of last year in West Virginia, was found to be caused by an obscured methane gas sensor. The United Mine Workers, which recently published a report of its investigation, claims that Massey Energy was responsible for maintaining the safety of the mine and its workers at all times.

The report claims that if the methane gas sensor was not covered by a ventilation curtain, the sensor would have been able to detect high levels of the gas.

Man causes explosion and six-alarm fire

A 28 year-old man caused an explosion, and subsequently a six-alarm fire, in a failed suicide attempt. The man was living with a relative in an apartment complex in Medford, Massachusetts when he rigged the natural gas line and caused it to ignite.

Nearly 50 people have been displaced after fire, smoke, and/or water damaged left 24 apartments uninhabitable. The fire burned for over eleven hours before it was contained and ten people were injured as a result.

The man behind the explosion is currently in the hospital suffering from third-degree burns. He is expected to face charges of attempted murder and arson once his medical conditions improve.

Furnace believed to have caused explosion

A newly installed furnace is suspected to be the cause of a deadly explosion in a residential Kansas home. Investigators say that the furnace was installed just days before it exploded and killed a woman.

The 53 year-old homeowner along with her 23 year-old daughter and her friend are still in the hospital. The mother is in critical condition and her daughter and friend are said to be in stable condition. Emergency responders say the explosion leveled the house and threw debris more than a hundred feet away.

The Reno County Sheriff’s Department believes a propane tank leaking gas is what ignited the furnance.

Boiler blamed for small explosion in Buffalo

A small explosion at the St. Clare Apartments in Buffalo, N.Y.  was caused by a faulty boiler.

The apartments are located on Main Street and the fire was reported just after 9 p.m. One person was taken to the hospital as a result of the explosion.

He was a maintenance worker for the building and was admitted to Erie County Medical Center to recover after the accident. His burns were considered to be non-life threatening. The fire caused $25,000 worth of damage to the building.
